Membership is intended to provide ongoing value for established and returning clients while maintaining the same professional, individualized standards used throughout Reveille Botox & Beyond.
- Annual Membership: Membership fee is $99.99 per year. Benefits begin upon enrollment and renew yearly unless canceled.
- Individual Use Only: Memberships are individual and may not be shared or transferred.
- Good Faith Exam (GFE): A separate $35 annual Good Faith Exam is required for applicable services such as neurotoxin, dermal filler, and other qualifying treatments.
- Discount Limitations: Membership discounts may not be combined with other promotions, nurse discounts, lifetime memberships, or holiday specials unless explicitly stated.
- Referral and Birthday Credits: Credits are subject to current program terms and may only be used as permitted by clinic policy.
- Online Storefront Exclusion: Membership discounts do not apply to the online vitamin storefront unless explicitly stated.
- Clinical Judgment: Treatment candidacy, dosing, product selection, and treatment timing are always based on medical history, anatomy, and provider judgment.
